Hospitals/ACOs collaborate efforts among carefully selected partners with the goal of delivering high-quality efficient care to the patient. This session describes a proven successful inter-disciplinary program that enables the SNF to become a valued member of the group, and achieves desired results including minimal risk for re-hospitalizations for their patients and ACO partners. Clinical outcome expectations, documentation, and details of how to facilitate the required mindset change will be highlighted.

Objectives:
1. The participants will be able describe the goals, characteristics, and bench marks of a successful ACO partner
2. The participants will be able to identify the potential roles that therapy plays in reducing re-admissions, improving quality, and achieving preferred provider status
3. The participants will be able to describe an IDT approach and best practices needed to be successful within the ACO/hospital partnership

Robin Boyle, PT - VP of Operations, Premier Therapy
Robin graduated from Cleveland State University with a B.S. in Physical Therapy in 1989. She has over 30 years of clinical experience in geriatrics, specializing in neurological disorders and Alzheimer?s/Dementia rehabilitation. Robin has significant work experience in both clinical and operational management including MDS, CMI, Reimbursement, Programming, Compliance and Operational Analyses. Robin has been recognized as a Keynote Speaker at multi-state levels, and is considered to be a leading industry authority.

Julia Bellucci, MS, CCC-SLP, Director of Clinical Development, Premier Therapy
Julia graduated from Edinboro University of Pennsylvania with a B.A. in Speech and Hearing Disorders, and from Clarion University with a M.S. in Speech/Language Pathology. She has over 25 years of clinical experience in geriatrics, specializing in dysphagia, aphasia and cognitive-linguistic disorders.

CEs & Other Education Requirements:

1 CE is available for NHA and Nurses.
How much does it cost?

Facilities will pay one low fee for an unlimited number of participants. However, the number and access code is valid only for one phone line/computer at the location registered with the Association. Any additional phone connections calling in will be billed the individual session registration fee.

Members:
$55 (prior to 4/1/2020)
$75 (4/1/2020 and after)

Non-Members:
$130 (prior to 4/1/2020)
$175 (4/1/2020 and after)
